[Item] Wolfshead Helm not working

This is a (hopefully) clearer version of #3937

[Wolfshead Helm] is not working. This Item is the foundation of a power-shifting feral druid and easily contributes 10-15% to the feral's DPS.

Current:
You do not get energy when you shift into cat form.
You do not get rage when you shift into bear form.

Expected:
You get 20 extra energy when you shift into cat form.
You get 5 extra rage when you shift into bear form.

When shifting it should cast on yourself:
To cat form: Wolfshead Helm Energy
To bear form: Wolfshead Helm Rage or Wolfshead Helm Rage
