How is identity handled in the phenopacket model and ecosystem?

We need a good description or some recommendation for how identity is managed on the phenopacket side and how it relates to identity in FHIR. This is especially important if we want to preserve some sense of consistent identity between a phenopacket based system and FHIR.

This issue came up as I was trying to decide on how to manage identity in bundles. For now, we'll probably keep as much of the original phenopacket identities (from the id field) in FHIR identifier entries but create new FHIR id values when creating FHIR resources.
