Update various IG metadata and narrative text from previous work

Many metadata items (publisher, version, status, etc.) need to be updated based on the IG being a CIC IG going forward.

We also need to read through the narrative text to make changes as needed.

Adopt ID syntax of lower case with dashes. For example, "some-profile-id". We need to update our IDs, and remove hard coded canonical URLs so that Sushi can generate the correct full URLs. We might need to check for any Reference.reference updates as well.
