Add upload ease and visibility

Motivation

**NB relies on #4 being complete **

Forcing people to remember to push an upload button will likely result in lost data, especially if the network and connection can be spotty. Ideally, we'd be upload continuously and flag to the user visually how much of their patient queue is not yet uploaded.

This also removes the drag to acceptance and creates a significant "just works" benefit.
