Problem Description

I have tried using Flash Attention in this repo on a server with MI250x devices and found that training is not stable:

In the figure, all the fluctuating curves are with flash attention enabled, and the red stable curve is when flash attention is disable. The issue does not happen on NVIDIA devices with the original CUDA implementation of flash attention.
