Formalicious v2. MODx 3.0.1 | But after the last update.

Good afternoon! Thank you very much for the snippet and the app. But after the last update it stopped working. Throws out on - HTTP ERROR 500/
Can you suggest why this might be? Have you tested? Did you have any mistakes? Formalicious v2. MODx 3.0.1

I am getting errors like this.
