qw release adds appropriate data to tables in Word templates

Definition of Done / Acceptance Criteria

All tables within the Word template are reproduced in the output by qw release
Mail merge fields within tables in the Word template are appropriately filled in by qw release
Table rows should be reproduced sufficiently to put all appropriate data into the table.
Leftmost columns iterate more slowly than columns futher right.
Cells that would contain the same data as the cell above are instead merged.

Testing

Automated tests for each of the four sub-requirements above.

Documentation

The README.md needs basic requirements. A tutorial video and a tutorial document would be appropriate, but are not part of this Issue.
