Activation notification email not sending to customer

Hi there,

We have been using this plugin for a long time but have recently noticed that the users are not receiving an email to notify them when their account has been activated. Other emails are sending out just fine. We are using Magento version 1.4.1.1.

Notify customer of account activation is set to yes. Any help would be greatly appreciated.
