Allow renaming phenotypes in the merge addin

phenoptrReports has strict requirements for phenotype names. They must match between different projects, and the names of the positive phenotypes must end in +. In practice, this is not always the case due to variant spellings and omission of the +.

It would be helpful for the merge addin to show all the phenotypes in the project and give an option to rename each one.
