A method of self-identifying strengths and weaknesses, or areas of interest

I need a way to better gauge where students are at coming in so I can tailor my feedback and experience better to individual needs. I am able to get this organically, but I spend a solid two projects discovering what that really, truly is. How do I assess? Can I rerun this assessment at the end of the semester somehow? And how do I build a balanced assessment of skills from this course that recognizes the different strengths people may come in with?
