support provided.al2 build? #200

Does this support building with the provided.al2 runtime? Using ginadapter, I got an error when running sam build:

Builder Failed: stat /tmp/samcli/source/boostrap: directory not found

If I run it with a normal APIGatewayProxyResponse, the build succeeds."
