The following changes should be made for the Industrial Wastewater processing activity form:

• Methodology dropdown, selections Alternative Parameter Measurement Methodology and Replacement Methodology should have Description field set as mandatory with respective notes as specified in this spreadsheet
• Measured conversion factor under the Source Oil-water separators acceptable values are only between 0-1
