Attach maps to approved RUPs

Some RUPs were approved before there were attachment options
Figure out way of attaching maps after approval
Need to identify that the map was attached after

This thought needs some work. We do need to fix those plans that were approved without a map. Figure that one out. Then ensure that the DM cannot approve a plan without a map. One is a data fix, the other is issue #1018
