Since we now have an XML Schema section, we don't need the separate Element Definition sub-sections anymore.

Instead, the elements should be explained where are referenced.
It should be clear where the definition of the elements is.
Rename XML Schema to "RELAX NG Schema" or make otherwise clear that RELAX NG (which should also be referenced) is used.
