The existing /provider-map page will be replaced by the new /for-vendors page which will be linked from the top nav on MobiMart home.

This can be implemented once #442 is complete.

Closing as won't do as we have the new Vendor page linked in the top nav and have the Provider Map page linked from Vendors.
