I would like to have some research around the CDC form that this project uses, I believe there could be an updated version.

This form looks more recent and has the July 31 2021 expiration date:

This form is closer to the one in the app: https://www.courts.phila.gov/pdf/forms/civil/CDC-Declaration-Halt-in-Eviction.pdf

the newer form has checkboxes in the place of bullet points, so we will need to render it with the checkboxes filled in
