guidance on NOT asking for things like patient demographics that should have already been available in CRD/PAS #39

I've seen several forms in CRD/DTR/PAS testing that include things like asking for the patient demographics, the procedure being performed, the associated diagnosis- which all should have already gone to the payer during CRD/PAS before determining that the DTR questionnaire needed to be filled out.

We should add guidance that these duplicative questions should not be included, and check that our examples do not have this issue.

I assume these fields are only present due to migrating paper forms as is, without considering the overall workflow context.
