Support cross development with target management

One of the main focuses for the CDT project is supporting cross development. This include cross build environments as well as remote launching and target management. The Eclipse Launch Bar was a huge help in that effort.

We'll need to add such features to the vscode extension in a manner that fits well within the vscode user experience.
