Users of Slack-IRC seem to experience a crash when ops add or remove a ban or quiet.
I'm not a user of Slack and I will never be so I can't help throubleshoot much further, but this sequence has become quite common to see in IRC channels when slack-irc is involved.

Do you have a stack trace or something from the bot when it crashes? I tried reproducing this, but nothing happened when I added/removed +b from users in channels the bot was in.
