Normalize event actions

Right now we have multiple versions of submitted/rejected/accepted event actions, depending on the entity that the event applies to, e.g. assertion submitted|rejected|accepted, change submitted|rejected|accepted and submitted|rejected|accepted (for evidence items). We should normalize these to simply submitted|rejected|accepted since the entity can be determined by the subject of the event.
