Add new contact modal

Here is the component we want to use from our framework - https://material-ui.com/components/modal/ (let's go with the transition modal)

• text fields https://material-ui.com/components/text-fields/ with validations
• select https://material-ui.com/components/selects/ for the dropdown for contact type which should call the GET all contact type endpoint

@jerryliangxia what did you have in mind with the add button next to the Job Title section? I don't think a pop-up over a pop-up is very UI friendly so what if we just add some text fields if the add button is clicked?
