Form only shows "submit" button

This is really cool! Nice work!

I am just having problems getting the form to work. The submit button displays and submits to Google Forms when pressed, but none of the fields appear in my HTML, so it submits empty values.

Thank you! Only accepting "short answer" inputs at the moment; implementing more input types in the next update.

I changed all of my fields to "short answer" fields, but they still do not show up in my HTML. Just the submit button.
