Revert "Ubuntu: ZFS has been promoted to main" This reverts commit a9b77b76aca6a395f3f0592f443ea9f58972a7a4. Apparently, zfs-initramfs has not been promoted to main.
Revert "Ubuntu: Disable hole_birth" This reverts commit 929d761ff3fbf37736e34ed4726653ee7d643798. I'm not convinced that disabling hole_birth is better. It may, in fact, be worse.
Remove eth0 Ubuntu uses the new naming scheme. Forcing the user to figure out the correct name avoids them blindly typing eth0 and ending up with broken networking.

Drop efivars This does not seem to be necessary. It failed when I just did a UEFI install. I tested again and the efivars filesystem is mounted by default in the live environment.
Recommend rootdelay for mpt2sas issues I haven't tested this, but it should address the problem as described.
Reorder GRUB steps For UEFI installs, /boot/grub/x86_64-efi/zfs.mod does not exist until grub-install has been run.
Add KVM UEFI instructions These came from Eric Villard <[email protected]>. Closes #4764
Ubuntu: Use /proc/self/mounts /proc/mounts is a symlink to /proc/self/mounts these days. The latter is recommended.
Update the GRUB ZFS module instructions When doing a UEFI install, the module is in /boot/grub/x86_64-efi instead of /boot/grub/i386-pc.
