Certain validation rules should be added to the assessment form.

if Sinkhole, a confidence level must be chosen.
at least one basemap must be checked
if Other, the comment field cannot be empty

The sink evaluation form is not currently operational, so these will have to be addressed when it is fully re-implemented.
