MathML support

• elementary math tags: mstack, mlongdiv, msgroup, msrow, mscarries, and mscarry.

• alignment groups in tables

• annotation-xml (to include non-mathml content, e.g., svg , (in particular in epub?))

• complete table attributes (e.g., columnspan and rowspan)
