Use a Transaction Bundle to Create Measure and Library Resource

In #9 we switched from transaction bundle to two single requests because HAPI did not support referencing the Library resource by its canonical URL. However SmileCDR only supports referencing by canonical URL and HAPI support for CQL is currently broken anyway.

DoD (updates pending):

• use a transaction bundle to create both the Library and Measure resource in one request
• use the url of the Library resource to reference it in the Measure resource
• extract the Measure resource ID from the transaction bundle response as already done before Properly support HAPI using CQL #9
