Consolidating everything (go-micro, go-plugins, examples, protoc-gen-micro)

I am thinking about consolidating all things into the single go-micro repo. This is more for clarity than anything else. It feels like both micro and go-micro have gone through these transitions and if go-micro is to live on, it doesn't feel like it should live as separate repos especially as a solo maintained project. For this reason, I'm thinking to move all things into go-micro {go-plugins, examples, protoc-gen-micro}

The consolidation has begun. You'll see new dirs examples, plugins and cmd/protoc-gen-micro. I'm slowly going through and doing major cleanup and some refactor. I'm trying to figure out what we support out of the box by default vs what are plugins that people pick and choose.
