Consider exposing internal thoughts and tool calls as messages to the runtime.

@gagb @husseinmozannar Thoughts? I can see this being useful for UIs and tracing tools, but probably should not be exposed to other agents in the runtime. Is is enough to do this via the structured logging infrastructure, or do we need more?
