Remove the UTF-8 BOM by Default #66

When a file is encoded as UTF-8 with BOM, the tool includes the BOM at the beginning of each code block it produces. This is incorrect behavior.

To fix this, the app should check for a BOM at the beginning of each file and remove it if one is present.
