Please provide at least documentation how to install.

Oh, as any other plugin. What do you need to know? You can either:

Download zip from github and unpack it in plugins/ folder in you LimeSurvey installation.
Go to plugins/ and issue git clone https://github.com/olleharstedt/ExtraQuickMenuItems

Then go to Plugin Manager and activate and configure which buttons you want to use.
