Detection submissions are sometimes duplicated

Every so often, it looks like detections get duplicated on submission:

I'm not sure if this is a frontend or backend issue. We might want to use some sort of unique index.

I ran into this on the reports page as well so am running the data through a quick dedupe function. The duplicates appear in the raw JSON so I assume this is a backend issue.
