Prototype Typeform for Volunteer Input form

@seanbae @colinking - I've been investigating TypeForm a bit more and I am liking what I am seeing. My thought is that I could put together a proto-type TypeForm in their normal web experience.

If I did that, do you know the level of effort involved in recreating the typeform in our app?

Our plan will be to use the Typeform UI to recreate the submission form flow within a standard Typeform, and then test this out with a handful of Botswana users and get feedback on how it compares to the current submission/edit flow.
