ENH: doctest-requires-all to declare dependencies for all doctests

In some cases it would be useful if dependencies could be stated upfront for the whole rst file rather than one-by-one for the code blocks. (afaik it's currently not possible, but I maybe wrong (and then the docs and tests should be updated here to cover that case).
