Add manual FHIRpath -> core mapping table #297

Rather than worry about trying to dynamically build this out, we should just manually create a table of the following structure:

fhir_path|table_name|column_name

This will allow us in the future to do things like 'backport to a future SQL on FHIR spec' or 'convey sources for data columns in counts'
