[feature] --codimd style generator without lowercasing the words #185

When trying to use doctoc with a codimd https://demo.codimd.org/ markdown document which renders an HTML document (either side by side or as "view only") it failed to work because the auto-generated anchors keep the uppercase caracters, so # Introduction gets you an #Introduction anchor, not #introduction as is generated by github and others.

I've filed an issue on codimd hackmdio/codimd#1590 but I guess this could also be changed on doctoc's side.

Based off that thread, it sounds like maaaaaybe that's a thing on codimd side? I don't know what that system does.
