Create configurable bypass mode

Need to create a way (via configuration) to shortcut the entire Duo processing and return success as if we successfully processed a Duo authN.

Use Case:

Campus total internet outage prevents users from loading the Duo iframe. A live-way to put the IdP into Duo bypass would be used to allow work to continue until internet is restored
