v15: Document Picker: Wrong Icon and Missing URL

Bug summary

From an RTE, if you search for a document in "Link to Document":

1. There's no URL below the results, so you have no idea which node it refers to. That's fine if you have one page with this name, but if you have multiple pages with the same name it's not a useful search.
2. The original icon used for the document is not visible, it's always the "document" icon.

@madsrasmussen With this issue, I have a fix for the search result item icons, but will need to clarify with you about the URLs (for document items).
