[✨ SAVE TEMPLATE 1/2] Update Template metadata fields when saving a new template

Acceptance Criteria
On /new-template/save, there should be 3 metadata fields required that the user fill out

• Template Name: required, text field
• Facility: required, text field
• Condition: required, text field
• Do not allow user to save template if any of the above fields are empty / null
