- The UDS3-A2 form primarily focuses on the co-participant's demographics, including birth month and year, sex, ethnicity, race, education level, and relationship to the subject. It also inquires about how long the co-participant has known the subject, living arrangements, and the frequency of in-person and telephone contact.
- In contrast, the UDS4-A2 form emphasizes the co-participant's assessment of the participant's memory and cognitive health. It includes questions about the relationship duration, living arrangements, frequency and duration of contact, and the co-participant’s reliability. Moreover, it asks about the co-participant's perception of the participant's memory, frequency of memory issues, and a comparative assessment of the participant's memory compared to 10 years ago.
- These differences highlight a shift from gathering basic demographic data in UDS3 to a more focused evaluation of the participant's cognitive health from the perspective of someone close to them in UDS4.
