docs: 更新版本信息和变更日志

- 将 README 和 CHANGELOG 文件中的版本号从 1.0.12 修改为 1.0.13
- 更新 CHANGELOG.md 中的版本比较链接
- 移除 README-zh.md 和 README.md 中的多余空行
