Update references.bib

Added White2005 reference on handling missing baseline covariates. PS: I should also have commented that the FDA guidance citation in the earlier FAQ version referred to the draft guidance and this statement has been removed in the more recent final guidance (which declares missing covariates simply as out of scope).
