Reproduce issue:

Assume the conference is on a different server to the out dial task.

  1. Initiate an outbound call to a UA.
  2. Return a conference verb from the webhook.

Behaviour:

REFER fails and call is left hanging.

Expected behaviour:

The call is moved to the server that hosts the conference.

#90 should cover the fix, I couldn't get it to work without the changes. Feel free to close if it's been addressed outside of my PR.
