Initial & bug fix release

This is the initial posted release of the Scientific Software Design book archive since the move to GitHUb. Posting this release eliminates the need for cloning the archive via git. The release also contains the most recent bug fix to eliminate a redundant synchronization in the coarrayBurgers solver when num_images()==2. Now the sync images statement now has only one argument to prevent redundancy in the image number list, which is a standard violation.
